In my cabin at AMD Dental Clinic, I hear a recurring sentiment that deeply concerns me: "Doctor, cap galat hai… isiliye dard ho raha hai" (The cap is wrong, that’s why it hurts). There is a pervasive myth that a dental crown is a permanent failure if it starts hurting. But let’s clear the air: The dental crown is just a cover. If you have a book with a torn cover, you replace the cover; you don't throw the book away. Your tooth is the book. The dental crown is just the protection.
1. The "Book Cover" Analogy: Understanding the Anatomy
To understand why your tooth might be acting up, you first have to understand what a crown actually is. Think of your tooth as a precious, antique book. If the pages are crumbling (decay) or the spine is cracked (fracture), we perform a "restoration"—often a Root Canal Treatment (RCT)—to stabilize the internal structure.
Once the "book" is repaired, we put a high-quality "cover" on it to protect it from the daily wear and tear of chewing.
The Crown is external: It sits on top of the tooth to strengthen and restore function.
The Issue is internal: Problems usually stem from the foundation (the roots) or the environment (the gums).
If the cover gets dirty or doesn't fit right anymore, you change the cover. You don't necessarily have to burn the book.

If discomfort starts immediately, the issue is likely related to the procedure itself:

Incomplete RCT: If the root canal wasn't cleaned to the very tip, residual bacteria can cause persistent sensitivity.
Improper Fit: A poor seal at the gum line (margin) can irritate the tissue or allow early leakage.
Scenario B: The Two-Year Turning Point
If your tooth cap was perfect for two years and then starts hurting, the crown didn't suddenly "turn bad." The environment changed.
The Perio Lesion: Over time, if oral hygiene isn't meticulous, food particles get lodged between the crown and the neighboring tooth. This leads to gum inflammation and bone loss.
Secondary Decay: Bacteria are microscopic. If there is a tiny shift in the gums, food can get trapped, causing a new cavity underneath the old work.
Root Fatigue: An old root canal can fail after years due to a microscopic fracture in the root itself.

They might say, "The tooth cap is the problem; we must remove it and pull the tooth." The Harsh Reality:
Hiding Mistakes: If that practitioner performed the original RCT, they find it difficult to admit the internal cleaning was inadequate. It’s easier to blame the "tooth cap."
The "Broken Tooth" Trap: Removing a dental crown requires precision. Inexperienced hands often apply too much force, which can fracture the underlying tooth during removal. To cover their tracks, they tell the patient the tooth was "rotten," justifying an extraction.
4. The Golden Rule: Both are Treatable
I want to instill one message: Pain does not equal tooth loss. Almost every issue—whether it’s a failed RCT or a gum infection—is treatable if you catch it in time. We can:
Re-Root Canal Treatment (Re-RCT): Cleaning and sealing the canals again using modern microscopes.
Gum (Periodontal) Treatment: Deep cleaning to treat infection caused by food lodgement.
Crown Replacement: Placing a fresh, better-fitting "cover" once the foundation is healthy.
